# Retatrutide vs Tirzepatide vs Semaglutide: Trials Compared (PH)

Single, dual, or triple agonist: how semaglutide, tirzepatide and retatrutide compare in their main obesity trials

Retatrutide, tirzepatide, semaglutide. Which one produced the most weight loss in trials? Here is the side by side. The big difference is how many receptors each one targets.

Semaglutide is a single agonist: GLP-1 only. Tirzepatide is a dual agonist: GIP and GLP-1. Retatrutide is a triple agonist: GIP, GLP-1, and glucagon. Now the trials.

Each one enrolled adults with obesity, or overweight with a weight-related condition, without diabetes. STEP 1, published in 2021: semaglutide, 68 weeks, 14.9% average weight loss, versus 2.4% on placebo.

SURMOUNT-1, 2022: tirzepatide, 72 weeks, up to 20.9%, versus 3.1%. TRIUMPH-1, results announced in May 2026: retatrutide, 80 weeks, up to 25%, versus 3.

9%. One caution. These are separate trials, with different lengths and different people. They are not head to head.

Side effects were mostly digestive in all three: nausea, diarrhea, and vomiting. In TRIUMPH-1, 11.3% on the highest dose stopped because of side effects. Some also reported dysesthesia, an unusual or unpleasant skin sensation.

Status: semaglutide and tirzepatide are approved prescription medicines in the US. Retatrutide is still investigational, with a US filing planned for early 2027.
