Why does weight loss on GLP-1 medicines stall? Filipinos researching these drugs ask it often. A plateau is when the weight curve flattens. In the big trials, every curve does it.

In semaglutide's STEP trials, weight loss appeared to plateau around week 60. A 2025 analysis of SURMOUNT-1 found about nine in ten people on tirzepatide had reached a plateau by week 72. Why? The first named mechanism is metabolic adaptation.

A smaller body burns less energy, sometimes even less than predicted. In a 2016 study of 14 contestants from a televised weight-loss competition, resting metabolism was still about 500 calories a day below predicted, six years later. Appetite pushes back too. In a 2011 diet study of 50 adults, leptin stayed lower and hunger stayed higher one year after weight loss.

So what happens after stopping? In the STEP 1 extension, published in 2022, people who stopped semaglutide regained about two-thirds of their lost weight within a year. In SURMOUNT-4, published in 2023, adults who switched to placebo regained 14 percent of body weight over the next year. Those who continued lost another 5.
