Semaglutide or tirzepatide? It is a debate you see all over Philippine weight-loss groups. One trial compared them directly. Both are lab-made versions of gut hormones called incretins.
Semaglutide is a single agonist. It activates the GLP-1 receptor only. Tirzepatide is a dual agonist. It activates GLP-1 and a second receptor, GIP.
Think one key versus two. The trial is SURMOUNT-5, published in the New England Journal of Medicine in 2025. It randomized 751 adults with obesity, and without diabetes, to one or the other for 72 weeks. Average weight loss was 20.
2% on tirzepatide, versus 13.7% on semaglutide. Waist size fell 18.4 centimeters versus 13.
And 31.6% lost at least a quarter of their body weight, versus 16.1%. The limits: it was open label, so participants knew which drug they got.
It was sponsored by the maker of tirzepatide. And it ran in the United States and Puerto Rico. We found no head-to-head trial in Filipino participants. Side effects were mostly digestive in both groups, mostly mild to moderate, and mostly while doses were being increased.
Status: semaglutide was first approved in the US in 2017, tirzepatide in 2022, both for type 2 diabetes, with weight management approvals in 2021 and 2023.
