In Russia, this peptide is on the national list of essential medicines. In most of the world, almost nobody has tested it. Semax is a synthetic chain of seven amino acids. It borrows a fragment of ACTH, a pituitary hormone, and adds a stabilizing tail of three amino acids: proline, glycine, proline.

It was developed at the Institute of Molecular Genetics in Moscow, and is used there as nasal drops. The mechanism researchers point to most is BDNF, brain-derived neurotrophic factor, a protein that supports nerve cell survival and learning. In a 2006 rat study in Brain Research, Semax raised BDNF in the hippocampus and improved learning in the animals. In a rat model of stroke, a 2021 study found changes in proteins linked to inflammation and nerve cell survival.

In humans, a 2018 Russian study of 110 people recovering from ischemic stroke reported higher blood BDNF and better motor recovery with Semax. It was not a blinded, placebo-controlled trial. What is missing: large randomized trials outside Russia, and long-term safety data. Semax is approved in Russia.
