Tirzepatide Explained: SURMOUNT, SURPASS and Philippines Status

Tirzepatide explained in 90 seconds: how the dual GIP and GLP-1 agonist works, what the SURPASS and SURMOUNT trials found, and where it is approved. Research notes

Tirzepatide is already an approved prescription medicine. So why do so many people in the Philippines still search for it as a peptide? Tirzepatide is a dual agonist: one peptide that activates two gut hormone receptors, GIP and GLP-1. Both hormones help regulate insulin release and appetite.

Picture two keys built into one molecule. The research comes from two large trial programs. SURPASS studied type 2 diabetes. In SURPASS-2, a 2021 trial in 1,879 adults with type 2 diabetes, every tirzepatide dose lowered A1C more than semaglutide over 40 weeks.

SURMOUNT studied weight. In SURMOUNT-1, published in 2022, 2,539 adults with obesity or overweight and no diabetes were followed for 72 weeks. At the highest dose, average weight loss was 20.9%, versus 3.

1% on placebo. Side effects were mostly digestive: nausea, diarrhea, constipation and vomiting, generally mild to moderate. In SURMOUNT-1, about a quarter to a third of people on tirzepatide reported nausea. Research on very long-term use continues.

Status: the US FDA approved tirzepatide for type 2 diabetes in 2022 and for chronic weight management in 2023. The European Union authorised it in 2022 and now covers both uses. In the Philippines, it is sold in licensed pharmacies as a prescription medicine. Research-grade vials are not that approved medicine.
